You can use this form to file an incident report with the Flame Ontario Conduct and Consent Committee. This form collects information regarding possible Code of Conduct violations that have occurred at SideBurn.
The submitted form should contain sufficient detail for the team to begin an investigation. Any details shared through this form are only available to the members of the Conduct and Consent Committee. In the event of identified conflict of interest a designated person from the Flame Ontario Board will be assigned. The committee will actively work to provide a more impartial and meaningful process to recommend remedial action, conditional participation, or exclusion of people at official SideBurn events based on reports and makes recommendations to the Flame Ontario Board, which retains the responsibility for implementing those recommendations as deemed appropriate.
